DENTAL IMPLANT BILLING

Dental Implant Billing Services

One implant case can run through two insurers and several billable stages. We coordinate coverage, predeterminations, and each phase, so every stage is billed to the right plan and less of the case slips through unbilled.

THE IMPLANT CHALLENGE

Why Implant Billing Is More Complicated Than It Looks

Most dental procedures are one visit, one code, one payer. Implants are none of those. A single case can include an extraction, a bone graft, a sinus lift, the placement, months of healing, an abutment, and the final crown, and the coverage rules change from one phase to the next. Some surgical stages may qualify for medical coverage when medical necessity, documentation, and the plan's policy support it, while the restoration is billed to dental. On top of that, dental plans add obstacles built for implants: missing tooth clauses that exclude teeth lost before coverage began, and alternate benefit rules that pay toward a cheaper option like a denture.

Getting these cases billed well means knowing the rules going in and planning around them, not discovering them after a claim comes back. Billdontix manages the full case across both payers, so each phase is handled in the right order with the documentation each plan expects.

  • Alternate Benefit RuleDowngrades implant coverage to pay for lower-cost dentures/bridges.

Where Practices Lose Implant Revenue

Implant cases are high-value, so the gaps here matter. They tend to show up in a few places.

01

Missed Medical Billing

Some surgical stages, like bone grafts, sinus augmentation, and placement, may be eligible for medical billing when the diagnosis, medical necessity, documentation, and plan policy support it. Send the whole case to dental without checking, and that potential reimbursement can be missed. Our dental-to-medical cross coding specialists review which phases may qualify and bill them to medical when they do.

02

Alternate Benefit and Missing Tooth Denials

Dental plans limit implant coverage with alternate benefit clauses and missing tooth rules, so a case gets downgraded or denied on a technicality. We check for these before treatment, so the patient understands their likely cost and the claim reflects what the plan actually covers.

03

Predeterminations Skipped on Big Cases

Skipping a predetermination on an expensive case invites surprise balances and stalled treatment. We request pretreatment estimates from both plans up front. A predetermination is not a guarantee of payment, but it gives the patient a realistic number and cuts down on write-offs later.

04

Phases That Fall Through the Cracks

Claims filed months apart are easy to lose track of, and a benefit year can reset mid-treatment. We track the full case from start to finish, submit each phase on time, and time claims around benefit years so less gets dropped or wasted.

FILING STRATEGY

How We Decide Medical vs. Dental

Surgical implant phases aren't automatically billable to medical, and routing them there by default causes problems. Before we bill a phase to medical, our certified coders review the diagnosis and whether it's tied to trauma or a medical condition, the specific procedure and how the plan classifies it, the patient's medical and dental plan policies, and the documentation available to support medical necessity. If the phase qualifies, we cross-code it to medical with the required CPT and ICD-10 codes. If it doesn't, it stays on the dental claim. The aim is an accurate claim, not a medical claim by default.

  • Diagnosis & trauma linking verification
  • Plan policy & procedure classification
  • Medical necessity documentation check
  • CPT/ICD-10 cross-coding suitability
END-TO-END BILLING

Our Dental Implant Billing Services

01

Benefit Verification and Predetermination

Before treatment, we verify both medical and dental benefits, check missing tooth and alternate benefit clauses, confirm remaining maximums, and request predeterminations, so your team can quote a realistic number before the first appointment. Predeterminations guide the estimate; they don't lock in final payment.

02

Medical and Dental Cross-Coding

We split each case across the right payers, coding eligible surgical phases to medical with CPT and ICD-10 and restorative phases to dental with CDT. Our certified coders review eligibility first, so each phase is coded to the plan that actually applies.

03

Surgical Phase Billing

We accurately bill all crucial surgical stages to the correct insurance plan. This encompasses tooth extractions, vital bone grafting, sinus augmentation, and actual implant placement. We always include the strict medical necessity documentation that health insurance claims demand for optimal claim approval.

04

Restorative Phase Billing

We expertly manage the restorative aspects of treatment as well. Our team bills abutments, implant crowns, protective bridges, or complete dentures directly to dental insurance. Everything is coded precisely and carefully reconciled against the patient plan coverage for the finalized dental restoration.

05

Denial Management and Appeals

Whenever an expensive implant claim faces an unexpected denial or an unfair downgrade, our dedicated denial management specialists quickly intervene. We meticulously identify the root cause, correct any submitted errors, and aggressively file formal appeals with extensive supporting clinical documentation to secure payments.

06

Payment Posting and Reconciliation

We diligently post cleared payments from both medical and dental plans across every single phase. We consistently match these amounts against your specific office fee schedules and proactively flag any hidden underpayments, ensuring your high-value implant cases are always paid correctly.

Frequently Asked Questions

Often a mix of both, which is why coordination matters. Some surgical phases can be billed to medical when medical necessity, documentation, and the plan's policy support it, while the abutment and crown are usually dental. We bill the case across both plans so the medical portion isn't overlooked. What's ultimately covered depends on each plan.

An alternate benefit clause, sometimes called least expensive alternative treatment, lets a dental plan pay toward a cheaper option, like a denture or bridge, instead of the implant. We check for these clauses and missing tooth rules before treatment, so the patient understands their likely out-of-pocket cost and the claim reflects what the plan will actually pay toward the case.

No. We request predeterminations on implant cases because they give the patient a realistic estimate and reduce surprises, but a predetermination isn't a guarantee. Final payment still depends on eligibility at the time of service, documentation, and the plan's rules.

No. Surgical phases like grafting, sinus augmentation, and placement can qualify when the diagnosis, medical necessity, documentation, and plan policy support it, but they aren't automatically medical. Our certified coders review each case, and when a phase doesn't qualify, it stays on the dental claim.

We track the whole case from the first phase to the final restoration. Each stage is submitted on time, and when a case crosses into a new benefit year, we time claims to use both years' benefits where it helps, so a phase is not dropped just because it was filed months after the one before it.

Yes, a BAA is available on request. We work inside your existing system using role-based access, individual logins, and audit logs, with HIPAA-aligned handling. You keep full ownership and visibility of your data, with no migration and nothing new for your team to learn.
